Seven amino acids. Built at the Russian Academy of Sciences in the 1980s from an ACTH fragment, then given a tail so enzymes would stop eating it in minutes. The goal was simple: keep the brain effects, drop the cortisol. They did that part.

What most Semax pages still get wrong is the job. This is not Adderall in a vial. The strongest human data is still stroke recovery in Russian hospitals. What Is Semax?

The name is Russian shorthand for “seven amino acids.” Sequence: Met-Glu-His-Phe-Pro-Gly-Pro. Developed in the 1980s at the Institute of Molecular Genetics, Russian Academy of Sciences, under Igor Ashmarin, later carried by Nikolai Myasoedov.

It is a synthetic analog of ACTH(4-10). Full ACTH tells your adrenals to dump cortisol. The middle fragment, Met-Glu-His-Phe, keeps a lot of the brain activity without that hormonal shove. The problem was time. Enzymes shredded the short fragment in minutes. Adding Pro-Gly-Pro to the tail blocked carboxypeptidase and bought the molecule a usable half-life. That tail is the whole invention.

Russia and Ukraine registered it as a prescription drug decades ago. It hit Russia’s List of Vital and Essential Drugs in December 2011. Hospital use there: ischemic stroke, TIA, cognitive disorders, optic nerve atrophy, encephalopathy. None of that is an FDA approval. In the US it has no approved indication, no USP monograph, and research vials are sold for laboratory use only.

Biohackers found it in the mid-2010s and never put it down: foreign pharma history, no stimulant edge, and a BDNF paper. Most of the human evidence is still Russian-language work that Western Phase III never copied.

How Semax Works

The mechanism everyone can agree on is BDNF and its receptor TrkB. BDNF is the growth-factor most tied to plasticity, learning, and keeping neurons alive.

The paper to know is Dolotov et al., 2006, Brain Research. One intranasal dose in rats: hippocampal BDNF protein up 1.4-fold, TrkB phosphorylation up 1.6-fold, BDNF mRNA (exon III) up 3-fold, TrkB mRNA up 2-fold. Learning on a conditioned avoidance task moved with the chemistry. A companion 2006 paper showed specific binding in basal forebrain.

The rest of the stack, shorter:

  • Other neurotrophins. After ischemia, Semax and the PGP tail upregulate NGF, NT-3, TrkA, TrkC (Shadrina 2010).
  • Dopamine and serotonin. Rat work shows a fast dopaminergic bump and striatal 5-HIAA rising for hours after one dose.
  • Enkephalinase. Like Selank, it slows the enzymes that chew enkephalins. Mild analgesia in some animal models.
  • Inflammation. Stroke models drop IL-1β, TNF-α, IL-6 (Medvedeva 2014).
  • Neuroprotection. Animal ischemia papers keep showing smaller infarcts and better survival under hypoxia and glutamate stress.

Oral Semax is a waste of a vial. Peptide bonds die in the gut. The published human studies and almost all of the animal work used the nose. That is how Russian hospitals gave it, and that is how Dolotov measured the BDNF jump. It is a study-design fact, not a rule for how a lyophilized research vial has to be used. Community practice with these vials is subcutaneous, same insulin syringe as every other peptide in the fridge. Oral capsules with the Dolotov paper taped underneath them are still the wrong product.

Even the people who study this molecule will tell you the pathway is not one switch. Neurotrophins, monoamines, melanocortin receptors, enkephalinase. Complementary, unfinished. Anyone selling a single-pathway story is compressing 40 years into a tweet.

Where that range came from

300–600 mcg is the middle of the Russian outpatient cognition charts, usually 10–14 day courses. Those charts were intranasal because that is how the drug was formulated there. Forums copied the milligram range, then most people with a lyophilized vial moved it to a subcutaneous pin. A chunk of users drifted toward 600–900 because 300 felt light. Hospital stroke protocols went to 1,200–1,800 mcg/day. Different job.

Side Effects

Published hospital papers noted nasal discoloration in some patients because those protocols were intranasal. On July 24, 2026 the Pharmacy Compounding Advisory Committee voted to recommend Semax free base and Semax acetate for the 503A Bulks List, 8–5–1. Same two-day meeting also recommended BPC-157, TB-500, KPV, MOTS-c, and Epitalon. Emideltide did not get a majority.

A committee vote is not a final rule. FDA still has to accept, edit, or reject it. Until that rule lands, US compounding pharmacies are not cleared to treat Semax like a routine 503A bulk, and research vials stay research vials. Do not read “PCAC said yes” as “your local pharmacy can make this tomorrow.”
